This report is required whenever a person is involved in any kind of incident or altercation at Soup Angels. The report should be completed by the person witnessing the incident or receiving the complaint, and/or investigating the incident.
This form must be filled out completely and electronically submitted to the Steering Committee and Board of Soup Angels within 24 hours of the incident. Please answer all questions. If an item is unknown, do not leave it blank. A member of the Steering Committee and/or the Board of Directors shall review this form and will be printed out and filed in Soup Angels records. A copy will be furnished to Soup Angels' legal counsel only if necessary.
